How to Fix Accountability Problems in Your Organization
Most accountability problems aren't accountability problems. They're clarity problems with accountability symptoms.
When accountability is failing, the temptation is to add controls, more reporting, more reviews, more pressure. It rarely works. Real accountability runs on three things: clear expectations, ownership the person actually accepted, and a culture where missing the bar gets named, not buried.
The fix usually starts further upstream than the leader thinks. Often, the person who can't be held accountable was never properly given the work in the first place.
